Welcome to my office. It is my professional policy to:

Provide you with the best service available based on my training and nearly three decades of post-graduate experience.

Promote comprehensive service delivery.

Coordinate clinical care with other qualified health professionals where needed and appropriate through direct referrals.

I use strict procedures for confidentiality that require completion of detailed forms authorizing the release of information that requires your signature.

Please take a few moments to review the following practice expectations. It is important that you:

Take an active role in your counseling and treatment planning via appreciating that honest and transparent disclosure will promote the therapeutic process.

Honor the confidentiality of other clients you may encounter.

Please give at least 24 hours advance notice of cancellation of an appointment.

Report any changes in your condition as soon as possible.

Take time to review and understand your rights as a recipient of service. You will be provided a summary Recipient Rights brochure for your reference.

Abstain from the use of alcohol and other mood-altering substances and take only medications that are prescribed by your attending physician. It is very important to have your therapy coordinated with your attending physician.

Please pay your bill for professional services at the time of the session unless other arrangements have been made with your primary therapist.

Established patients will be given an after hours contact phone number.

Listed below is the current fee schedule:

Individual /Couples Therapy - $100.00 per 50 minute clinical hour

Group Therapy - $50.00 per individual, per 90 minute session.

Assessments - $100.00 per clinical hour (including report compilation)

Interventions - $100.00 per 50 minute clinical hour
